Edmonds: A Practical Manual of Diabetic Foot Care 2nd Edition

The first edition of 'A Practical Manual of Diabetic Foot Care' was named ‘Medical Book of the Year 2004’ by the British Medical Association, and we were pleased on behalf of all workers in this difficult field that the importance of the diabetic foot has been thus recognized and acknowledged as a major area of interest.

A new edition is an opportunity to present revised and updated material, together with many new photographs and case studies and descriptions of fresh developments in the field.

In this second edition, the author still emphasize the importance of the simple staging system, as a fundamental part of the approach to the management of the diabetic foot. By applying the staging process to all diabetic foot patients it is possible, through one simple assessment, to detect the conditions that need addressing and plan their treatment using simple but fundamental principles.

The simple staging system has therefore been developed as a practical, treatment-based tool that provides a framework for diagnosing and managing the diabetic foot, and this chapter describes the application of this system through:
  • Practical assessment, which consists of history-taking, examination and investigations.
  • Classification of the diabetic foot into the neuropathic and neuroischaemic foot.
  • Staging the foot. The six specific stages in the natural history of the diabetic foot.
  • Management plan for each stage.

The clinical assessment we describe is quick and simple, but will enable the practitioner to make a basic classification and staging and the foot can then be placed into the correct stage, which has specific implications for treatment. Investigations will be needed to assess severity so as to determine treatment.

Following this system will enable the practitioner to make rapid, effective decisions that will detect problems early, organize rapid treatment and prevent deterioration and progression.

All diabetic foot enthusiasts will enjoy this new edition of this Practical Manual, and will find it as useful, practical and easy to use as its predecessor.

  • Dr Michael E. Edmonds MD, FRCP, Consultant in Diabetes, King's College Hospital, London, is a world-renowned expert on foot problems in diabetes. He has lectured on the topic throughout the world and gave the inaugural Roger Pecoraro Lecture at the American Diabetes Association's annual meeting. He is also Chairman of the Diabetic Foot Study Group of the European Association of the Study of Diabetes.
  • Mrs Alethea V. M. Foster BS (Hons) PGCE, MChS, SRCh, DipPodM recently retired as the Chief Podiatrist, King's College Hospital, London, wher she was involved in the development of the UK Government's National Serviec Framework for Diabetes implemented in all hospitals, clinics and primary care practices form 2004. In May 2003, Alethea was the first podiatrist to be awarded the International Diabetic Foot Award, which is only awarded every four years to an indvidual who has played a pivotal role in developing diabetic foot care.
  • Dr Lee Sanders DPM, Chief of Podiatry, Acute Cre/Specialty Services, Department of Veterans Affairs Medical Center, Lebanon, PA, USA. Book Review
The third edition of Ian Scobie’s Atlas of Diabetes Mellitus, building on the success of the two previous editions, goes a long way towards making this task easier and remains a testimonial to the fact that a picture is indeed worth at least a thousand words. This Atlas begins with a brief overview of the diagnosis, pathogenesis and treatment of diabetes with special reference to the complications of diabetes and the treatment of children and adolescents. Specific chapters cover diabetic dyslipidemia, diabetes and pregnancy and what it is like to live with this chronic condition. These, and other sections, set the stage for the multitude of highly informative illustrations which follow. Most specialists in diabetes have seen real-life examples of the disease processes presented in this book but some may be very unfamiliar to the generalist.
Many excellent and comprehensive textbooks of diabetes are available, but I am unaware of any other illustrated text with the scope and breadth of Dr Scobie’s Atlas of Diabetes Mellitus. I have no doubt that both my students and I will continue to find this book useful for many years to come. I suspect that those of you who choose to add this excellent Atlas to your library will find that you will also share my enthusiasm for this delightful book.

Ahima: Metabolic Basis of Obesity

Overweight, defined as body-mass index (BMI) greater than 25, and obesity, defined as BMI greater than 30, are characterized by excessive fat accumulation which poses adverse risks to health. Worldwide, there are more than one billion overweight and 300 million obese adults. Obesity is a major risk factor for diabetes, cardiovascular disease, sleep apnea, non-alcoholic fatty liver disease, arthritis, cancer and other diseases, and has been associated with enormous health costs, premature death, reduction in the quality of life and disability. The increasing global incidence of child obesity is also a major concern. The obesity epidemic is not just a problem for industrialized societies. Indeed, obesity rates have risen three times or more over the past two decades in some developing countries. Although the growing obesity epidemic undoubtedly reflects profound changes in diet and lifestyle over recent decades, genetic factors are important in determining a person’s susceptibility to weight gain and adverse health consequences of obesity.

Ain: The Complete Thyroid Book 2nd Edition

Roughly 12 percent of the adult population suffers from some form of thyroid disease. The most common thyroid diseases are autoimmune thyroid diseases, which affect women much more frequently than they do men, especially when women are pregnant or after delivery. These comprise Hashimoto’s disease (see Chapter 5) and Graves’ disease (see Chapter 6). Immunoendocrinology: Scientific & Clinical Aspects (Contemporary Endocrinology Series)

As illustrated by the chapters in this book, autoimmune disorders are diverse and the same. At the heart of these diseases are specific alleles of genes of the major histocompatibility complex favoring the targeting of specific antigens, with diseases differing in the specific alleles favoring or preventing autoimmunity. The fine balance between self tolerance and autoimmunity is upset by major mutations in a few families, and multiple polymorphisms in most families. Large scale genetic (research involving the screening of hundreds of thousands of newborns) and then immunologic prediction of many autoimmune disorders is now a reality for both families and the general population. With the exception of celiac disease, environmental factors essential for disease and contributing to the marked increase in many of these disorders are undefined. Despite our lack of comprehensive knowledge, multiple immunotherapeutic clinical trials are underway.

It is my hope that our current knowledge base for Immunoendocrinology presented here will prove sufficient to design and test preventive paradigms. Nevertheless, it is my belief that our current basic research knowledge, also presented here, is insufficient to be assured of disease prevention with current trials. Contained within each of the chapters are incomplete drafts of maps that should eventually lead to disease prevention for specific disorders that in combination provide a comprehensive draft of our knowledge and ignorance of the continent of autoimmunity.

Each fi gure corresponds to a case treated in our Diabetes Centre in the Athens University Medical School. It is no exaggeration to state that the new (second) edition of this book resulted from the marked success of the first edition. It must also be noted that the problems of the diabetic foot continue to be a major and very serious concern of every society in the world, even those with the best healthcare systems.

Thompson: The Low-Starch Diabetes Solution: Six Steps to Optimal Control of Your Adult-Onset (Type 2) Diabetes

Until recently, doctors thought adult-onset diabetes was the same as childhood diabetes—that both were caused by lack of insulin. In the 1980s, scientists made the remarkable discovery that they’re entirely different diseases. Whereas kids with diabetes lack insulin, most adult-onset diabetics make plenty of insulin—often more than normal. The problem is that their bodies lose sensitivity to it. Now doctors refer to the disease that young people get as type 1 diabetes and to the kind that middle-aged adults get as type 2-diabetes. This book is for people with adult-onset, or type 2 diabetes.

In times past, the diagnosis of diabetes had tragic implications. Children and young adults with type 1 diabetes often wasted away and died from it. Insulin, when it came along in the 1920s, was the miracle drug for these patients. It allowed them to live normal lives. In those days, the troubles of middle-aged and older folks with type 2 diabetes seemed minor compared with young people with type 1 diabetes. Adult-onset diabetics could live for years with hardly any treatment at all. They could usually get their blood sugar down to reasonable levels by just taking some pills and watching their diet. Doctors rarely prescribed insulin for these patients; they fi gured it wasn’t worth the trouble.

In the 1990s, new research showed that while patients with type 2 diabetes rarely died as a direct result of their diabetes, their mildly elevated blood sugar levels—if these went on long enough—could result in damage to their eyes, kidneys, and blood vessels, and the attendant increase in heart attack rate was alarming. As a result, doctors started taking adult-onset diabetes more seriously, treating it as they would type 1 diabetes, using stronger pills and insulin when necessary to get blood sugar levels as close to normal as possible.

In 2008, researchers released the results of two large studies on the effects of this more vigorous approach to treating adult-onset diabetes. The results were disappointing. Heavier doses of insulin helped patients avoid eye and kidney damage but did little to reduce the rate of heart attacks. What worked for patients with juvenile diabetes didn’t work as well for people with adult-onset diabetes. Those results are not really surprising, considering that type 2 diabetes is a completely different disease from type 1. It has different causes, results in different complications, and requires a different approach to treatment. The goal of treating type 1 diabetes is simple: replace the missing insulin. Type 2 diabetes is more complicated. The body becomes resistant to the effects of insulin, which not only raises blood sugar but brings on cholesterol, blood pressure, and weight problems that cause as much trouble as the high blood sugar does. This often requires a multipronged approach to treatment.

The good news is that you can live to a ripe old age without suffering any health problems from your type 2 diabetes. When the nineteenth-century sage George Bernard Shaw said the secret to good health is to get a chronic disease and take good care of it, he might as well have been talking about adult-onset diabetes. When you do what you need to do to treat it, you reverse many of our modern lifestyle’s harmful effects on your health and general well-being. Indeed, you might find yourself feeling better than you have for years.

  • Christopher Byrne trained as a physician and clinical scientist at Cardiff, Cambridge and Stanford Universities. After Medical Research Council clinical science fellowships he was appointed Professor of Endocrinology & Metabolism at Southampton University Hospitals Trust, UK. He is an academic diabetologist with clinical and molecular research interests in the pathogenesis of the metabolic syndrome, and the mechanisms linking metabolic syndrome with type 2 diabetes and cardiovascular disease. His publications span a range of disciplines from epidemiological to gene expression studies. He has had 112 articles published.
  • Sarah Wild trained in medicine in London and Cambridge and subsequently received training in epidemiology at Stanford University and the London School of Hygiene and Tropical Medicine. She is currently senior lecturer in epidemiology at the University of Edinburgh. Her research interests include the global burden of diabetes and metabolic syndrome and cardiovascular epidemiology.She has had 31 articles published.
