The first edition of 'A Practical Manual of Diabetic Foot Care' was named ‘Medical Book of the Year 2004’ by the British Medical Association, and we were pleased on behalf of all workers in this difficult field that the importance of the diabetic foot has been thus recognized and acknowledged as a major area of interest.

A new edition is an opportunity to present revised and updated material, together with many new photographs and case studies and descriptions of fresh developments in the field.

In this second edition, the author still emphasize the importance of the simple staging system, as a fundamental part of the approach to the management of the diabetic foot. By applying the staging process to all diabetic foot patients it is possible, through one simple assessment, to detect the conditions that need addressing and plan their treatment using simple but fundamental principles.

The simple staging system has therefore been developed as a practical, treatment-based tool that provides a framework for diagnosing and managing the diabetic foot, and this chapter describes the application of this system through:
  • Practical assessment, which consists of history-taking, examination and investigations.
  • Classification of the diabetic foot into the neuropathic and neuroischaemic foot.
  • Staging the foot. The six specific stages in the natural history of the diabetic foot.
  • Management plan for each stage.

The clinical assessment we describe is quick and simple, but will enable the practitioner to make a basic classification and staging and the foot can then be placed into the correct stage, which has specific implications for treatment. Investigations will be needed to assess severity so as to determine treatment.

Following this system will enable the practitioner to make rapid, effective decisions that will detect problems early, organize rapid treatment and prevent deterioration and progression.

All diabetic foot enthusiasts will enjoy this new edition of this Practical Manual, and will find it as useful, practical and easy to use as its predecessor.

About the Author
  • Dr Michael E. Edmonds MD, FRCP, Consultant in Diabetes, King's College Hospital, London, is a world-renowned expert on foot problems in diabetes. He has lectured on the topic throughout the world and gave the inaugural Roger Pecoraro Lecture at the American Diabetes Association's annual meeting. He is also Chairman of the Diabetic Foot Study Group of the European Association of the Study of Diabetes.
  • Mrs Alethea V. M. Foster BS (Hons) PGCE, MChS, SRCh, DipPodM recently retired as the Chief Podiatrist, King's College Hospital, London, wher she was involved in the development of the UK Government's National Serviec Framework for Diabetes implemented in all hospitals, clinics and primary care practices form 2004. In May 2003, Alethea was the first podiatrist to be awarded the International Diabetic Foot Award, which is only awarded every four years to an indvidual who has played a pivotal role in developing diabetic foot care.
  • Dr Lee Sanders DPM, Chief of Podiatry, Acute Cre/Specialty Services, Department of Veterans Affairs Medical Center, Lebanon, PA, USA. He is past President for Health Care and Education of the American Diabetes Association, the first podiatrist in ADA's history to be elected to this prestigious position. Dr Sanders is also a medical historian, with a special interest in the history of diabetes.
