In the 12 years since the publication of the first edition of Urologie Surgical Pathology, great strides have been made in our understanding of urologic diseases. Many clinically important new histopathologic entities have been described or more fully defined in virtually every genitourinary organ. Numerous clinically important diagnostic and prognostic markers have entered routine practice, including alpha-methylacyl-CoA racemase, the most widely discussed biomarker in surgical pathology in this century. Genetic testing for the early detection of bladder and prostate cancer and the molecular classification of renal cell carcinomas has become increasingly important. This is an age of enlightenment in urologic surgical pathology, and we have attempted to capture the sense of excitement herein.
Our specialty of surgical pathology continues to evolve. The emergence of personalized medicine has created a paradigm shift in diagnostic and prognostic biomarker analysis. Diagnostic tests are now linked to individualized treatments. The dream of digital imaging is slowly being realized; high resolution images are now routinely captured and transmitted across the internet for multiple purposes. Central pathology review of clinical trial cases has become common practice, and central review protocols will likely be standardized in the near future. The accuracy of predictive outcome models for individual patients continues to be improved. New algorithms, nomograms, neural networks, and other models emerge virtually every month. Further, evidence-based practice with an emphasis on levels of scientific evidence is replacing the ‘intuitive’ practice of medicine, resulting in more consistent outcomes.
Throughout this renaissance in the classification and diagnosis of urologic diseases and in the prediction of individual patient outcome, the histopathologic examination of tissue and cells remains the gold standard. Today's surgical pathologist in any part of the world finds urologic specimens to be an increasingly important part of everyday sign-out. The expectation for excellence in diagnosis remains very high. It is with this understanding that we have produced our second edition.
This book is intended chiefly for use by surgical pathologists in their daily practice. The numerous comments and suggestions from readers of the first edition have been carefully considered and incorporated into the current effort. The text has expanded more than 30%, and the number of fullcolor illustrations now exceeds 1200. Two new chapters have been added (Urine Cytology and Fine Needle Aspiration of the Kidney) to reflect increasing emphasis on cellular diagnosis in urologic pathology. We have remained true to our purpose of providing a framework for comparison, evaluation, and integration into clinical practice of the evolving urologic diagnosis. The emphasis remains on the practical aspects of diagnostic pathology.
