McKenna: Atlas of Minimally Invasive Thoracic Surgery (VATS) Expert Consult Series with DVD

We think that minimally invasive thoracic surgery is good for patients and our specialty. The evidence is now overwhelming that operations that previously required thoracotomy incisions can now be performed safely by video-assisted thoracoscopic surgery (VATS), without compromising the efficacy of the operation. Compared to a thoracotomy, VATS offers patients comparable or reduced morbidity and mortality, earlier return to regular activities, earlier discharge from the hospital, and an overall lower cost. All thoracic surgeons need to understand this and learn the techniques of minimally invasive thoracic surgery, regardless of where they are in their careers, in order to offer these benefits to our patients. The purpose of this atlas is to enable thoracic surgeons to accomplish this transition from open surgery to minimally invasive surgery.

Atlas of Minimally Invasive Thoracic Surgery (VATS) is designed to be a detailed “how to” manual for VATS procedures. The initial chapters present basic concepts of VATS techniques, and the bulk of the remaining chapters focus on step-by-step details of the most commonly performed advanced VATS procedures. Accompanying the book is a DVD with videos of the procedures corresponding to each chapter. The videos are also available on the book's companion website at www.ExpertConsult.com.

Manual of Thoracic Endoaortic Surgery

In today’s healthcare environment, the real driver is value. Ashcraft's Pediatric Surgery 5th Edition (Expert Consult Series) with DVD

This is the fifth edition of Pediatric Surgery, but the first edition without the insights and efforts of either Dr. Tom Holder or Dr. Keith Ashcraft. In 1980, these two master surgeons had the foresight to develop a reference text focusing on infants and children with surgical conditions. The current editors would like to dedicate this fifth edition to our founding editors, Drs. Tom Holder and Keith Ashcraft.

Drs. Holder and Aschraft wanted to create a single-volume pediatric surgical work that was broad in scope and had some detail about each condition; however, they did not intend for this work to be encyclopedic. As they stated in their first Preface, “An effort has been made to provide sufficient information concerning physiology, embryology, and anatomy so that the reader may gain a thorough understanding of the patient, the disease process, its symptoms, diagnosis, and treatment. Authors were selected because they are acknowledged authorities in the area of their contribution.”

This concept of a single-volume text has persisted over the subsequent editions. The current editors have tried to maintain this book in the style originally conceived by Drs. Holder and Ashcraft. Although most of the chapters in this edition have the same title as those in the first edition, the information and knowledge in the field of pediatric surgery have dramatically evolved. More than half the illustrations in this fifth edition are new. Management of urologic conditions remains an important part of this book because many pediatric surgeons around the world also perform urologic operations in children. An effort has been made to use experts from all over the world to write the chapters, as this book is read worldwide by surgeons, pediatricians, residents, and medical students.

Many, if not most, operations are now performed using a minimally invasive approach. Thus, operative technique is emphasized more in this edition than in previous ones. Prenatal diagnosis is an entity now commonly encountered by pediatric surgeons. In addition, so much more about genetic predisposition to certain conditions and cancers is known now versus 30 years ago. Finally, in this fifth edition, a chapter on “Evidence Based Medicine” has made its way into a pediatric surgical textbook for the first time.

As previously emphasized, a concerted effort has been made to keep this book to a single volume. However, as there is so much more information that is important and known about pediatric surgical conditions than was available in 1980, we also need to allow for that growth. In an effort to limit the book to a single volume, references have been placed online on the associated Expert Consult website (www.expertconsult.com), rather than at the end of each chapter in the book itself. While some readers do not look at the references, those who do will find that they are easily accessible and linked to PubMed for easy access to abstracts and full text. By saving nearly 200 pages of text, we have helped to maintain the cost of the book. Also, the amount of paper needed was significantly reduced. In addition, the full text and color figures are also available on Expert Consult. Purchasers receive a PIN code and password with each copy of the book making this edition the first pediatric surgery text that is available online. We hope our readers enjoy and appreciate this feature as we believe that a definite advantage of this book will be its portability.     Podesser: New Solutions for the Heart: An Update in Advanced Perioperative Protection

    This interesting volume on advanced perioperative protection during cardiac surgery is an excellent compilation of past, present and future science of this very important and fundamental topic for every cardiac surgeon.

    The idea for this book was conceived by Bruno Podesser in 2006, after realizing that there had been no comprehensive summary of myocardial protection during cardiac surgery since a book edited by HM Piper and CJ Preusse in 1993 (Ischemia Reperfusion in Cardiac Surgery).

    The Editors, Dr. Chambers and Podesser have assembled an all star cast of excellent clinicians and scientists, experts in the general fields of basic cardiac research, cardioplegia research and clinical outcomes research. After an initial excellent historical chapter the various authors present much of the basic science of myocyte physiology, myocardial protection, subjects that have not been well understood. These basic concepts have been summarized beautifully in this book for clinicians and basic science investigators alike. The next section discusses much of what is currently known about the clinical and physiological effect of cardioplegia on the beating and arrested heart, the optimal protection for the transplanted heart, specific recommendations for right heart protection and a variety of modalities to achieve optimal protection in these situations.

    Finally, there are several chapters about the potential future of improved protective cardiac strategies during surgery for an increasing number of patients, particularly the elderly, who will require cardiac surgery over the next decades. The authors have done an excellent job in putting together the best of science and clinical practice of a subject which is critical to successful cardiac surgery and which was put into effect only 30 years ago with the institution of hyperkalemic cardioplegia. Cardioplegia has become a scientific and clinical requirement for excellence in heart surgery.

    Therefore, this update in advanced perioperative protection is urgently needed. The results and strategies presented in this book will expand our knowledge on injury and protection of the heart and thereby can improve the outcome of our daily work.

Others have special purposes, such as atlases on technique or algorithm based books on decision-making. All have their place, but if the educational goals are training of the young surgeon, self-assessment and continuing medical education for the practitioner or preparation for oral examination, this book fills a special need, and fills it very well by breaking away from the didactic approach.

    It has long been recognized by educators that retention of knowledge, i.e. true learning, are much better achieved using the Socratic method of questions and answers, as opposed to simply reading or being told facts. In this book this approach is developed and presented in a very effective manner. In each “chapter,” one is presented with a case report representing a real life scenario. The case reports-scenarios in this book together cover most of vascular surgery experience. Following the case report, one is presented with questions and answers based on various aspects of the case, forcing the reader to commit to an answer.

    Whether the answer is right or wrong is not critical, in fact getting a wrong answer may be more beneficial in terms of correcting knowledge and retaining information. The commentary and conclusions that follow analyze the choice of answers, correct and incorrect, and discuss them in concise, authoritative detail, many of which are truly “pearls of information.” The conclusion then summarizes the current state of knowledge on the clinical issues under consideration. Numerous references are included. Together, these components constitute one of the most effective vehicles for self-education in vascular surgery today. Importantly, all aspects of management are covered: diagnostic evaluation and appropriate treatment, whether it is non-operative or interventional, endovascular or open surgery.

    To accomplish their goals the editors have gathered together a large number of experienced contributors, many well-known for their special areas of interest within vascular surgery, reflected in the contributions they make to this book. As such, the book should be useful to future and practicing vascular surgeons all over the world.     Endovascular surgery and other catheter-based interventions are taking on an increasingly important role in the management of patients with vascular disease. Vascular surgeons and interventionalists alike are recognizing the importance of this therapeutic modality and are constantly seeking training and updates to fully make use of these treatment options in their daily practice. When the first two editions of this book were prepared, there were a limited number of vascular surgeons with sufficient experience to contribute chapters. Since then, the situation has changed considerably. In the third edition, as well as the current fourth edition, every chapter has been written by a vascular surgeon, thus emphasizing the emerging role of the vascular surgeon in this important and expanding field. Currently, it is estimated that endovascular surgery represents 50% to 70% of the average vascular surgeon’s practice.

    The field of endovascular surgery has progressed significantly since the third edition. The fourth edition reflects this progress and maturation. For example, the promising laser angioplasty and atherectomy technology that was discussed in the second edition was dropped in the third edition because of disappointing results; however, it is reintroduced in the fourth edition to reflect recent clinically relevant developments. Aortic stent grafting has taken on an even more important position, particularly concerning fenestrated endografts and the combined open and endovascular hybrid techniques for thoracic and abdominal aneurysms and dissections. In addition to complete updates of prior chapters, interventional management of superficial and deep venous disease has been greatly expanded.

    The book has kept its major sections, which include general principles, imaging, and the various therapeutic modalities. Sections are also broken out anatomically to address the variety of endovascular techniques appropriate for each anatomic location (such as the aortoiliac system, the infrainguinal arteries, the visceral arteries, and the supra-aortic trunks). The management of specific problems (such as vascular graft thrombosis, endografting for aneurysms and traumatic injuries, dialysis access salvage, and venous surgery) are updated with real and promising developments. There are also added a new section of representative instructional case presentations, featuring integrated clinical and angiographic data.     This book is designed to meet the needs of those who are first entering the field as well as experienced endovascular surgeons who wish to update their skills and have access to a current database of results. To accomplish these objectives, the text covers the basic technical aspects of a variety of procedures in all anatomic locations, except for of heart and intracranial circulation. Individual chapters are prepared to stand alone so that the text can be used as a data resource. When several interventional options are available for a given lesion, the immediate and long-term results are compared and the advantages and disadvantages of each technique are discussed. Finally, pharmacologic adjuncts and methods to prevent intimal hyperplasia and late failure are addressed.

    It is the editors’ expectation that this book will be comprehensive and up-to-date and will serve the needs of the vascular specialist for several years to come.

    Little: Complications in Cardiothoracic Surgery: Avoidance and Treatment 2nd edition

    The reception of the first edition of this book has encouraged to proceed with the development of this second edition. As before, this book is a complement to the standard cardiothoracic surgery textbooks that address operative techniques and postoperative care. Acquisition of the information gained from the careful study of these books helps to establish the basic fund of knowledge that the thoracic surgery trainee and practitioner builds upon to establish final surgical competence. This knowledge foundation is supplemented by the real world experience with patients in the operating room, hospital, and clinic. It is this combination of knowledge and experience that leads to the development of the fully mature and capable thoracic surgeon. 

    This real world learning process involves the recognition of complications and the ability to learn from them. This is expressed in the common observation that good results come from experience and experience is acquired by making mistakes and learning from them. As before, the goal of our book is to minimize the frequency of surgical complications and maximize the patients’ outcome when they do occur by allowing the reader to learn from the operative and clinical experience of those who have gone before. This means that each generation can learn and benefit from the experience already gained by others. There is no need for each of us to make our own mistakes or have our own complications if we can benefit from the experience and accumulated wisdom of others. 

    Therefore, this book and its chapters are focused on the issue of complication prevention and/or recognition and treatment. While the chapter authors have been asked to address the correct or standard way to perform operations and care for patients afterward, they have also been challenged to address and emphasize specific issues related to both intraoperative techniques and postoperative care that will reduce the incidence of complications. This is a slightly but importantly different focus from standard textbooks. As some complications are essentially inevitable, also addressed by the authors are the issues of timely recognition and appropriate treatment of complications when they do occur, despite best efforts to prevent them.
     
    In summary, his book will serve as a useful supplement to, and not a replacement for, standard textbooks and operative atlases.

    Patterson: Pearson's Thoracic and Esophageal Surgery 3rd Edition (Expert Consult Series) 2 Volume Set

    The current edition of these two volumes is substantially changed from the previous two editions. One important change is the title. The previous editions were entitled Thoracic Surgery and Esophageal Surgery. The current volumes are renamed Pearson's Thoracic and Esophageal Surgery. This change acknowledges the enormous impact Dr. Pearson has had on developing, practicing, teaching, and research in the discipline we have come to know as general thoracic surgery. Dr. Pearson was the senior editor of the first two editions of this text, and, with his appointment as honorary editor to this edition, the editors decided the change in name was timely. It should be noted that virtually all of the authors of the previous and current editions have been students, partners, or colleagues of Dr. Pearson or were themselves trained by one of Dr. Pearson's trainees.

    This edition was developed and executed by a new editorial board. With the untimely death of Dr. Robert Ginsberg and the retirement of Drs. Harold Urschel and Clem Hiebert from the editorial board, we were given an opportunity to add illustrious new members. Dr. James Luketich brings an international reputation for the development of minimally invasive surgery and other innovative techniques in thoracic surgery. Drs. Tom Rice and Toni Lerut have both made major contributions to the field of esophageal surgery. They shared principal responsibility for editing the esophageal volume. Of course, Dr. Lerut also brings an important international perspective to this edition and was able to recruit a number of international experts as authors. As in prior editions, Drs. Joel Cooper and Jean Deslauriers continue to make important contributions to the thoracic volume. It has been my privilege to serve as senior editor of this edition.

    The editorial board has reflected on the recent passing of two outstanding thoracic surgeons, both of whom were influential in the evolution of this textbook. In recognition of their contributions, the thoracic volume is dedicated to Dr. Robert Ginsberg and the esophageal volume is dedicated to Mr. Ronald Belsey. These volumes have not previously been dedicated, but the appropriateness of these dedications is evident by the tributes to these great surgeons that follow on subsequent pages.

    The content of these two volumes has changed dramatically to reflect developments in the 6 years since the last edition. The vast majority of chapters are new additions or prior topics rewritten by new authors. The authorship represents an excellent collection of expertise from North American and international contributors.

    These thoracic and esophageal volumes embody the enthusiasm and exciting developments that characterize general thoracic surgery internationally. The biology of diseases we confront is being clarified by outstanding basic and clinical research. Clinical staging of thoracic malignancies, although still not perfect, is much more accurate than only a few years ago. Innovative surgeons have developed and refined minimally invasive techniques for many operative procedures we perform routinely. National and international societies as well as an increasing number of institutions continue to commit to the development of general thoracic surgery. Postgraduate training and curriculum requirements demand attention to and focus on general thoracic surgery. Although we as practicing thoracic surgeons benefit from this progress, the ultimate beneficiaries are the young surgeons attracted to this exciting field and, of course, our patients.
