This video atlas covers the anatomy, basic techniques, and major procedures in liver, biliary, and pancreatic surgery. While not intended to be comprehensive, for example, transplantation and the surgery of portal hypertension are omitted, this work covers the majority of the operative procedures in the field.
The videos in this atlas have been gathered over some 12 to 15 years and are the edited records of operations carried out at Memorial Sloan-Kettering Cancer Center, New York, and reflect my personal approach to hepatobiliary and pancreatic surgery—in other words, how I understand the problems and “how I do it.” The text is intended to act as important background reading, and most of the drawings will be found reflected in the operative sequences portrayed in the videos.
The concept of this atlas is based on tutorials given to the fellows in oncologic surgery at Memorial Sloan-Kettering Cancer Center. These video tutorials always commence and conclude with a brief discussion of each procedure, its indications, and the operative problems likely to be encountered. I hope this atlas is viewed in this context and that the reader will dip into the descriptive text before viewing the related videos.
I have been told repeatedly by the fellows that, when they go out in to practice, these video tutorials provide a sound basis for their operative work in the specialty. I am confident that this will prove to be the case for surgeons in training and am hopeful that it will prove of value not only to the general surgeon who has the necessity to undertake hepatobiliary and pancreatic surgery but also to the established specialists in the field who might find agreements on some points but much to debate. Indeed, I realize that, while this work conveys my personal approach to hepatobiliary and pancreatic surgery, there are many other valuable techniques, and indeed every surgeon develops personal nuances and tricks of the trade.
I believe that this work offers you, the viewer and reader, a safe, reliable, anatomically based approach to a complex field of surgery. It is not possible to cover all the nuances of diagnosis, to teach surgical judgment, and to show the full range of complications, but these videos provide a solid background of knowledge that will allow further understanding of the procedures involved. Finally, these videos will allow the development of skills in the correct timing and sequence of the steps during the various operations and in the development of surgical judgment.
While my personal contribution to this atlas is entirely related to open operative procedures, I have asked Dr. Carlos Corvera of San Francisco to contribute a video section on laparoscopic techniques in the management of liver disease and pancreatic tumors. Dr. Covera's laparoscopic operative techniques have been based on the procedures in open surgery that he developed during his training as a fellow at Memorial Sloan-Kettering. The translation of open techniques to the laparoscopic arena is obvious and beautifully displayed. The mark of a trained laparoscopic surgeon with expertise in liver, biliary, and pancreatic problems should be based on full knowledge of open operation and the ability to convert from the laparoscopic to the open operative mode. Dr. Corvera displays this confidence. I am most grateful for his contribution.
Hepatobiliary and pancreatic disease is now an established subspecialty of general surgery. I am fortunate and privileged to have been part of this development.
