Orkin: Oncology of Infancy and Childhood (Expert Consult Series)






The decision to create a separate oncology volume for the seventh edition of Hematology of Infancy and Childhood was not difficult. Many readers of the sixth edition view pediatric hematology and oncology as one discipline, but nevertheless had to look elsewhere for comprehensive information on the nonhematologic cancers of childhood. Previous efforts to bridge this gap with a single, lengthy chapter on solid tumors fell woefully short, because of the exponentially expanding base of knowledge, which simply could not be captured in the pages we had formerly allotted. Thus, David Fisher and Thomas Look were charged by our coeditors, Stuart Orkin, David Nathan, David Ginsburg, and Sam Lux, and by our publishers and editors at Elsevier, especially Dolores Meloni and Ann Ruzycka Anderson, to expand the existing chapters on hematologic malignancies in Hematology of Infancy and Childhood and complement them with new chapters on malignant solid tumors and the supportive care of childhood cancer patients.

We believe the final product, Oncology of Infancy and Childhood, justifies our decision to launch a new textbook of this scope. The reworked format, although retaining updated and indispensable chapters from the sixth edition, is structured to accommodate emerging research developments in cancer biology and therapeutics, both globally and in specific pediatric tumors. To preserve the marriage of clinical practice to basic pathophysiology so central to the success of earlier editions of the Nathan and Oski textbook, we recruited a mix of clinical investigators as authors who we felt would maintain a consistent balance between fundamental disease processes and the principles of effective management of cancer patients. The inclusion of in-depth molecular pathophysiology provides more than an academic curiosity. As with pediatric hematology, many of the molecular-genetic lesions first discovered in childhood cancers gave birth to deep scientific insights of very broad importance in modern biology. And these lesions also are guiding the gradual transformation of oncology therapeutics toward a discipline guided by focused, molecularly-targeted therapies. At all times during the creation of this book, we remained keenly aware that the effects of cancer and its treatment are not restricted by time but extend from the earliest symptoms to the rehabilitation period and well beyond, leaving their imprint on both the patient and his or her family. To provide readers with an in-depth understanding of the issues raised by acute and chronic cancer sequelae in children, we have added a supportive care section written by authors who represent critical subdisciplines involved in this important branch of pediatric oncology.

This volume was organized and written to meet the needs of a diverse readership: clinical specialists and researchers who must stay abreast of a rapidly expanding information base; students, trainees, and junior faculty who are just learning about or entering the field of pediatric oncology; and practitioners in other disciplines who increasingly are called upon to recognize and deal with the delayed effects of treatment. It is our sincere hope that Oncology of Infancy and Childhood will earn a reputation as more than an educational tool. If we have performed our task well, this volume should also stimulate the next generation of physicians and biologists to tackle the many outstanding challenges presented by childhood cancer. Our greatest hope is that this volume will require frequent and major revisions, as new and improved therapies continue to reach our patients.
