Caviglia: Orthopedic Surgery in Patients with Hemophilia






Hemophilia A and B are lifelong inherited X-linked bleeding disorders, caused by the deficiency of factor VIII (FVIII) or IX (FIX) in plasma. The clinical hallmark of hemophilias is bleeding into the joints and muscles, which can occur spontaneously in the severe forms of the diseases (FVIII/FIX < 1 IU/dl). The joints more frequently involved are the ankles, knees, and elbows, and the recurrence of joint bleeding leads to the formation of ‘target’ joints. The final consequence of repeated hemarthroses is the development of hemophilic arthropathy, characterized by synovial hypertrophy, cartilage destruction, and bone damage with stiffness, pain, and ultimately permanent disability.

Severe hemophilia is a lifelong inherited bleeding disorder characterized by recurrent joint bleeding, which results in chronic, painful, and disabling arthropathy. Since the 1990s, the greater availability of safe and effective clotting factor concentrates has allowed satisfactory results to be achieved in hemophilia care.
Unfortunately, up to 70–80% of hemophiliacs worldwide, particularly in developing countries, still have no access to adequate replacement therapy and consequently frequently develop musculoskeletal complications with severe impairment of their quality of life. Therefore, the role of the orthopedic surgeon becomes crucial in the management of such patients.

This book aims to elucidate peri-operative and operative management of hemophiliac patients. It focuses on surgical techniques in order to give both fundamental and specialized information to those involved with hemophilia treatment worldwide, and to give useful advice to any orthopedic surgeon who is working in this challenging field.
