Maffulli: Tendon Injuries: Basic Science and Clinical Medicine

Standard textbooks of anatomy, physiology, pathology, orthopedic surgery, and sports medicine provide little information on tendons. Tendon ailments are increasingly prevalent in orthopedic surgery and sports medicine, and in occupational and family medicine as well.

This book provides a comprehensive presentation on human tendons for a wide range of readers, from students and teachers of physical education, biomechanics, medicine, and physical therapy to specialists such as orthopaedic surgeons, pathologists, and physicians specializing in sports medicine. This book describe the current principles of diagnosis, treatment, and rehabilitation of tendon injuries and disorders. Although  acknowledge that these principles are constantly changing, this book gives readers the tools presently available to the scientific and biomedical community to tackle tendon problems. This book has been conceived to be used as a comprehensive source for physicians, surgeons, physical therapists, chiropractors, sports coaches, athletes, fitness enthusiasts, and students in a variety of disciplines. The book is definitely a medical book, but with appeal to professionals outside the medical field.

The editors have collectively more than 70 years of experience in orthopaedic sports medicine, and have dedicated much of their research efforts to studying the pathophysiology of tendon problems. They believe that, as a team, our knowledge and experience will give help and guidance in the management of tendon problems.

In recent years—at least in the West—the demand for heavy physical work has markedly decreased. Conversely, leisure-time sports activities have become more popular, frequent, and intense. Repetitive work, excessive weight, poor fitness, and the lack of regular exercise and of variation in physical loading have all contributed to the increased incidence of degenerative changes in the musculoskeletal system. Tendon problems are seen frequently in nonathletes. Modern athletes also suffer from tendon ailments.The biological limits that musculoskeletal tissues can withstand are exceeded, with overuse and acute injuries, especially in tendons.

This book provides principles of diagnosis, treatment, and rehabilitation for various tendon problems. We envisage the book to be heavily used by physicians, surgeons, physical therapists, athletic trainers, and other professionals treating patients with tendon problems.

Sekiya: Techniques in Hip Arthroscopy and Joint Preservation Surgery (Expert Consult Series) with DVD

In Orthopedic Surgery, the sixties and seventies were dominated by teaching and studying outcomes of total hip arthroplasty. There was change in the focus of a majority of orthopedic surgeons from classical operations such as osteotomies, as postulated by Pauwels and his students, to the complicated subject of hip replacement with synthetic materials. These materials were studied to understand their behavior under conditions of motion and load.

Indeed, study of factors producing accelerated wear in artificial hip joints or causes of their frequent dislocations identified the phenomenon of motion-induced impingement caused by mechanical conflict between the components of the hip joint replacement. This observation led to design modifications of both femoral and acetabular components to avoid this occurrence. Understanding of this problem in the setting of total hip arthroplasty strongly suggested the possibility of the existence of this problem in the natural hip and in hips treated with osteotomy.

Indeed in the relatively small group of patients with dysplastic hips who had pain following periacetabular osteotomy, physical findings on examination, and radiographic evidence identified impingement between the femur and the acetabulum as the cause of these residual symptoms. Many had classical findings of impingement on the femoral head and characteristic acetabular labral damage at the time of re-operation.

This book is a much awaited reference on the details of these new concepts, including the very important subject of the role of arthroscopy in the management of these difficult cases.

Czervionke: Imaging Painful Spine Disorders (Expert Consult Series)

Our primary goal in writing this book was to provide a readily accessible reference guide describing the imaging appearance of the major conditions that cause back pain. This book deals with common and some not so common disorders that cause back pain, with emphasis on the imaging appearance of disorders that are associated with spine pain.

The book is organized into two major sections: Normal Computed Tomography and Magnetic Resonance Imaging Anatomy, and Painful Spine Disorders. An illustrated chapter is also included on the nomenclature currently used to describe the various types of disc herniations. The anatomic sections of the book include normal CT and MR images of the cervical, thoracic, and lumbar spine as well as the sacrum. Representative cross-sectional images are presented in these anatomic areas in axial, sagittal, and coronal planes.

The clinical disorder section of the book is divided into chapters. Each chapter topic is introduced by an actual clinical case of a patient with a given disorder who presents with back pain. Each chapter includes a brief description of the clinical presentation, imaging findings, and clinical course when relevant. The case studies illustrate the imaging presentation of these patients with back pain. A detailed description of the disorder, including relevant clinical and pathologic information, follows. Also included in each chapter is specific information about the imaging appearance of the particular pain-producing condition, emphasizing the radiographic, CT, and MR appearance, and a differential diagnosis with imaging examples.

The images pertaining to the case presentations are the first set in each clinical disorder chapter. They are followed by additional relevant images of the same or other patients with the same condition. These additional images are used to emphasize the range of imaging features possible for the particular topic being discussed. Images of the disorders in the differential diagnosis are also included if available or relevant.

Following the imaging features in each chapter is a brief discussion or listing of the major treatment options currently available for the particular disorder being presented. This book is not intended to provide an exhaustive discussion of all treatment options for back pain but rather is a practical guide that can be used to aid in the diagnosis of pain-producing spine and paraspinal disorders. Hopefully this information will be helpful in formulating a treatment plan for the patient with back pain.

Carefully selected references are included at the end of each chapter. We have chosen these references because we believe they supplement the information presented in the chapter discussions and provide the reader with a starting point for more detailed investigation regarding a particular pathophysiologic feature, clinical presentation, or imaging finding mentioned in the chapter. Our goal in preparing the third edition of Clinical Orthopaedic Rehabilitation: An Evidence-Based Approach is to widen the scope of available information for the musculoskeletal practitioner. The greatly expanded material should prove relevant to physical therapists, orthopaedic surgeons, family practitioners, athletic trainers, chiropractors, and others who treat musculoskeletal disorders.

We have attempted to provide evidence-based literature covering sound examination techniques, classification systems, differential diagnoses, treatment options, and criteria-based rehabilitation protocols for common musculoskeletal problems. With this material, the clinician who suspects de Quervain tenosynovitis of the wrist, for example, may easily look up the appropriate examination, differential diagnosis, treatment options, and criteria-based rehabilitation protocol.

Although the literature describing orthopaedic surgery techniques and acute fracture care is sound and comprehensive, there has been a relative paucity of information concerning nonoperative and postoperative rehabilitative care. This void exists even though rehabilitative therapy often has as much or more of an impact as the initial surgery does on the long-term results. A technically superb surgery may be compromised by improper postoperative rehabilitative techniques, which may result in scar formation, stiffness, rupture of incompletely healed tissue, or loss of function.

Skirven: Rehabilitation of the Hand and Upper Extremity 6th Edition (Expert Consult Series)

Synergy, in general, may be defined as two or more agents working together to produce a result not obtainable by any of the agents independently. Synergy is the ability of a group to outperform even its best individual member. The sixth edition of Rehabilitation of the Hand and Upper Extremity is the product of the synergy of editors, authors, publishers and many others involved in its publication.

The impetus for the first edition of Rehabilitation of the Hand grew out of a unique symposium that featured hand surgery correlated with hand therapy, sponsored by the Hand Rehabilitation Foundation in Philadelphia in 1976. The original editors of the book were also the chairpersons and faculty for the symposium: James M. Hunter, MD; Lawrence Schneider; Evelyn Mackin, PT; and Judith A. Bell Krotoski, OTR, FAOTA, CHT. Joining the effort with the second through fifth editions was Anne D. Callahan, MS, OTR/L, CHT. These extraordinary individuals introduced a working partnership of hand surgeons and hand therapists for the care of the hand patient that has endured and flourished over the years and is evidenced by the publication of the sixth edition of this book.

The expansion of this text and its readership is in keeping with the growth of the specialty of hand rehabilitation. This current two-volume edition features a total of 143 chapters, 37 of which are new, and more than 75 new authors. The authors of the text include physical and occupational therapists, certified hand therapists, orthopedic and plastic surgeons, physiatrists, neurologists, psychologists, psychiatrists, clinicians, researchers, and educators—all having expertise in the care of the hand and upper extremity patient.

Since the first edition, the table of contents has expanded with each edition to include separate sections on the shoulder, elbow, and wrist, as well as the hand. Many returning sections have been modified and expanded to reflect current practice. For example, the term orthosis is used to refer to the custom fabricated devices typically referred to as splints. Far from just a technical skill, the design and fabrication of hand and upper extremity orthoses require an in-depth knowledge of anatomy and pathology, as well as the healing and positioning requirements for the range of conditions and surgeries encountered. Hand, occupational, and physical therapists are uniquely qualified to design, apply, monitor, and modify orthotic devices as part of the rehabilitation treatment plan.

Taking advantage of the advances in information technology, this edition is complemented by a companion web site allowing supplemental information and video clips of therapy and surgery procedures to be included.

Given the emphasis on evidence-based practice in the current healthcare environment, special focus has been placed on providing peer-reviewed literature support for the information given in this text. However, published research in hand and upper extremity rehabilitation is limited in many areas. In some cases the best evidence is the clinical experience of the individual authors. Where it is stated that no evidence exists to support a particular approach or technique, the intention is not to suggest that it be abandoned; rather the goal is to stimulate the reader to adopt a critical attitude and to pursue clinical research, whether as a single case study or a multicenter randomized controlled trial.

We have dedicated this edition to Evelyn Mackin, who has been the driving force behind the book, as well as so many other groundbreaking achievements. Her leadership, dedication, determination, and inspiration have been instrumental in advancing the specialty of hand rehabilitation, as well as inspiring countless others (including the current editors) to follow her lead and further her initiatives. Available on the book's web site is a fascinating interview with Evelyn, recounting the early days of hand therapy, the formation of the American Society of Hand Therapists, the development of the Journal of Hand Therapy, and many other aspects of her extraordinary career.
