Mauro: Image-Guided Interventions (Expert Radiology Consult Series)

Image-Guided Interventions is an international endeavor that incorporates the work of authors from around the globe. From the outset, it was our intention to produce a body of work that is practical, concise, well illustrated, and representative of the state of the art of practice across the continents. To that end, the five editors were chosen based on their well-established expertise and global representation: Drs. Murphy, Venbrux, and Mauro (North America); Dr. Zollikofer (Europe); and Dr. Thomson (Asia-Oceania). The chapters are meant to be concise and practical and not encyclopedic. With Internet access to the world's literature, textbooks no longer need to be an exhaustive list of references. Image-Guided Interventions is designed by Elsevier to be a living document. It will be included in the new Elsevier initiative entitled Imaging Consult. Imaging Consult is an online diagnosis- and decision-support tool containing content organized by organ system, including interventional procedures. This book forms the basis for a major component of Imaging Consult, which will feature annotated images, high-yield content for quick answers to clinical questions, more extensive references linked to PubMed, patient cases with images focusing on the desired clinical topic, and links within the program to full reference content. Volumes in the Expert Radiology series will be a prominent component of Imaging Consult over the next 5 years.

The field of image-guided interventions is the result of the fortuitous marriage of two rapidly developing technologies: medical imaging and medical devices. Fluroscopy, ultrasound, computed tomography, and magnetic resonance imaging have made tremendous advances not only in their ability to produce diagnostic images but to guide intervention as well. The explosion in the number of medical devices that can be placed into the body through very small openings and guided with imaging has been particularly impressive over the past decade.

The text is divided into two basic parts: Vascular Intervention and Nonvascular Intervention. Part I: Vascular Intervention includes interventions that are performed via the vascular system (arterial, venous, and lymphatic). Part II: Nonvascular Intervention involves interventions performed via direct percutaneous access to the site of interest. The first six sections of Part I provide a base of core topics that are germane to much of the subsequent material dealing with vascular intervention. Chapter 1, on the history of angiography and intervention, written by Tony Venbrux and collaborators, gives reverence and recognition to our predecessors and pioneers in the field. The sections on Instruments of Intervention and Principles of Intervention review vascular devices and general concepts that are germane to much of the subsequent material. In a similar manner, Section 19 (the first section in Part II) begins with a discussion of basic devices and principles of nonvascular intervention.

Image-Guided Interventions is a textbook that is designed to be read either primarily from cover to cover or simply in preparation for a specific upcoming procedure. Indications and contraindications are reviewed at the outset to allow the reader to determine if the patient in question is a candidate for the procedure. The materials required for the procedure and the actual procedural steps are clearly listed in table form for easy reference. Elsevier made a commitment from the outset to the production of a well-illustrated and practical textbook. Readers will be impressed with the quality and clarity of the illustrations and drawings.

Image-Guided Interventions is a large undertaking with 27 sections and 168 chapters. The authors represent the best of the world's collective interventional minds, which are needed to produce a body of work that is state-of-the-art and relevant to practice. This book provides an extensive review of a wide range of procedures in interventional radiology illustrated through case examples. Although the cases may be studied in isolation, this book may also be used as a complement to the complete textbook, Image-Guided Interventions.

In 2008, Elsevier released, as part of the Expert Radiology series, a comprehensive, two-volume text on the field of interventional radiology, Image-Guided Interventions, edited by Matthew A. Mauro, Kieran P.J. Murphy, Kenneth R. Thomson, Anthony C. Venbrux, and Christoph L. Zollikofer. This textbook, written by many of the world's leading experts in interventional radiology, encompasses information about a wide range of procedures commonly performed in the interventional radiology suite in a state-of-the-art and practical format. The Teaching Files: Interventional illustrates many of these procedures as “real-world” examples.

I have attempted to bring together cases that illustrate both vascular and nonvascular interventions, combining customary techniques and novel approaches. Each case was identified through an exhaustive search of the University of North Carolina teaching file, looking for examples with instructive imaging and constructive clinical information. I did not shy away from cases with complications and adverse outcomes. Instead, these were included to show how these situations may present and be managed.

As the primary author and editor of this book, I would like to thank my colleagues who helped and supported me through this process and helped contribute to writing of several of the cases. In addition, past and present colleagues performed the majority of the included procedures, and again, I am thankful for their expertise. I would also like to thank the many former residents and fellows who participated in quite a few of these cases and from whom I have learned as much or more about interventional radiology as I have been able to teach.

In conclusion, this is a book about interventional radiology that teaches by examples. The reader may use this book in isolation or as an adjunct to the larger text Image-Guided Interventions.

Burke and Dixon: High-Yield Imaging: Interventional (Expert Consult Series)

High-Yield Imaging: Interventional is a comprehensive yet easily referenced handbook of the expanding field of image-guided procedures. It is a condensed and summarized version of the large two-volume textbook Image-Guided Interventions. High-Yield Imaging: Interventional covers a wide array vascular and nonvascular Interventions. There are 111 chapters within the vascular section, ranging from abdominal aortic endografting to percutaneous thoracic duct embolization. An additional 52 chapters comprise the nonvascular section. Eight chapters are dedicated to the complex management of vascular malformations. The burgeoning field of onco-intervention is extensively represented by 26 chapters of transarterial, transvenous, and endoluminal procedures as well as percutaneous thermal and chemical ablation therapies. There are even 10 chapters on head and neck interventions, including acute stroke management. Pain management procedures, an area that has been well established in Europe for many years, comprises the final seven chapters of the book.

Each chapter describes a procedure, beginning with a list of indications and contraindications. A list of the equipment necessary for the procedure is followed by a summarized description of the pertinent anatomy and approach. The technique to actually perform the procedure is then described in an easy-to-follow, step-wise manner. Controversies are reviewed that may require special considerations. Outcomes and complications are summarized, which is necessary for discussions with referring physicians as well as for informed consent. Each chapter ends with a review of the recommended post- procedural and follow-up care and a list of suggested readings.

Ray: Pain Management in Interventional Radiology

Everyone feels pain. Everyone feels physical pain, to one degree or another. Pain, or simply the thought of being in pain, often changes our actions as no other physical sensation can. While often a necessary physical response to keep us out of harm’s way (touching the hot handle of a pot, for example), pain becomes its own entity when it is associated with an underlying disease process.

Why is pain associated with disease? From an evolutionary perspective, was pain necessary for some reason to let the individual know that something was amiss (even though nothing could be done about it)? Was disease-associated pain used for some other, perhaps subconscious, purpose? Perhaps understanding the ‘‘why’’ is not all that important in today’s world; after all, the bottom line is that pain simply hurts! And disease-associated pain can hurt most of all. We as health care providers should be able to do something about it – shouldn’t we?

From one perspective, medicine (traditional or ‘‘Western’’ medicine in particular) has done relatively little to abate pain.Most of the major advances in pain control over the past 150 years have been in the field of pharmacology; general anesthesia is the prime example of pain control. With the exception of medications, however, little progress has beenmade inmanaging disease-associated pain over the past few decades. More work – much more work – remains. Shouldn’t minimaliy invasive techniques spearhead this effort?

As interventionalists become more involved with patients as care providers rather than solely as proceduralists, understanding and treating pain is a vital part of daily practice. This book provides an overview of the multiple techniques used in the management of pain in interventional radiology suites. Topics include techniques for the treatment and prevention of pain caused by interventional procedures, as well as minimally invasive techniques used to treat patients with chronic pain symptoms. Approximately half of the book is dedicated to the diagnosis and treatment of spinal pain; other chapters focus on intraprocedural and post-procedural pain management, embolization and ablation techniques used to treat patients with uncontrollable pain, and alternative treatments for pain relief. This book is a practical resource for anyone looking to acquire skills in locoregional or systemic pain control and wishing to improve the quality of life for patients undergoing procedures or suffering from disease-related pain.

The goal of this book is simple. It is to provide the appropriate tools to the interventional radiologist, anesthesiologist, surgeon, or whoever else is interested in minimally invasive techniques to control pain before, during, or after procedures. Its intent is to provide an overview of multiple techniques used in pain management, to review the currently available literature regarding these techniques, and hopefully to act as a springboard to motivate practitioners and researchers alike to develop the next better mousetrap to care for our patients in pain. It is my sincere hope that someone, somewhere, will have an improved quality of life stemming from the reading of this book by their health care provider.
