For more than two decades the Handbook of Patient Care in Vascular Diseases has provided health care providers at all levels a practical reference that emphasizes fundamentals and clinical concepts that “culminate in excellent outcome for vascular patients” (Darling C., First Edition). While the Handbook has a tradition of excellence, it finds itself in a new age of vascular care. Early editions were published in an era when information was exchanged at national meetings and in print and turned at a sluggish pace. At the time of the early editions training was dogmatic, methods of treatment changed gradually, and there were few natural history studies or randomized trials to direct vascular care.

Today there is near-instant access to a volume of clinical data often in the palm of one's hand before results are in print. Prospective trials and natural history studies guide the management of some vascular disease states, yet endovascular therapies are promoted on the Internet, in magazines and industry releases often without good peer review. Less invasive technologies developed by industry and favored by patients are introduced at a rapid pace. Practices and training paradigms have now evolved with an emphasis on such technology as well as proficiency through disease-specific clinical pathways.

This environment represents a challenge to those caring for patients with vascular disease. Today's provider must condense information from all directions sorting the relevant from the extraneous to develop a logical plan of care for each patient. In this setting we believe there is a role for an agile, evidence-based resource to serve as a discriminating guide and information management tool. Our hope is that this new edition of the Handbook represents such a resource. The Fifth Edition is structured to provide a basic understanding of vascular disease and cardiovascular risk rooted in the fundamentals of the history and physical exam. This edition incorporates noninvasive vascular testing and new clinical guidelines into the patient evaluation and provides a basic but thorough review of endovascular technologies and devices. Finally the Fifth Edition culminates in logical care outlines for eight areas of vascular disease encountered by trainees, nurses, and physicians.

With mentorship from two of the original authors we are privileged to continue this time-honored endeavor and hopeful that you will find the Handbook a useful resource to assist in the management of vascular disease.

Book Review
The fifth edition of the Handbook of Patient Care in Vascular Diseases inaugurates the transition from one generation of authors (Hallett, Brewster, and Darling) to the next (Rasmussen, Clouse, and Tonnessen). The change in authors recognizes the need to pass the torch to the younger men and women who will lead the care of vascular patients into the future.
The first edition of the Handbook dates to 1982, when open surgery was well established but endovascular therapy was relatively new. Basic principles of care remain the same. However, new endovascular options, proliferating cardiovascular medicines, and developing cell therapies required a reorganization of the previous Handbook.
The central purpose of the original Handbook remains unchanged: To serve as a primer of principles that optimize patient outcomes. The authors have revised every chapter and added the latest in diagnostic and treatment approaches. They provide concise and clear suggestions to every member of the care team. Both novices and more experienced practitioners, including cardiologists, radiologists, and surgeons, will find some thing in these pages to help them in their daily work.
