Since the publication of the first edition of this atlas, anesthesiologists, surgeons, and pain specialists—as well as patients—are increasingly acknowledging that peripheral nerve blocks provide an effective and safe alternative to either general anesthesia or neuraxial techniques for upper and lower extremity surgery; and that they represent an effective treatment for acute and chronic pain. Besides reducing the frequency of postoperative nausea and vomiting and allowing for earlier discharge, the use of these techniques has been associated with better outcomes while addressing the cost containment concerns of today's healthcare environment.
Continuous nerve block techniques play an important role in providing orthopedic patients with optimum postoperative pain control by improving functional outcome and reducing the length of hospital and rehabilitation time. The increased interest in continuous nerve block techniques also benefits from the realization that, contrary to epidurals, these techniques do not expose orthopedic patients to the increased risk of epidural hematoma if they receive potent anticoagulants postoperatively. These techniques benefit both in-patients and ambulatory patients, as well as adults and pediatric patients.

Although this edition has been completely revised, it is still based on the same principles of simplicity and practicability, using many color illustrations and photographs showing anatomic landmarks and patient positioning. For each block, there are the same presentation: patient positioning, indications, needle size, and volume of local anesthetics required, relevant anatomical landmarks, and description of the technique. In addition, an increased number of tips to maximize success and avoid complications have provided. The list of relevant references also have expanded and updated. When it is considered helpful, the relative positioning of the operator is also detailed. This edition also includes a more expanded section on the pharmacology of local anesthetics according to their indications (single blocks, continuous infusions techniques) and the patient population (adults and children). The anatomy chapters have expanded, since this knowledge is critical.
