Skarin: Atlas of Diagnostic Oncology 4th edition (Expert Consult Series)






Considerable progress has occurred in the fields of oncology and hematologic malignancies during the 7 years since the third edition of the Atlas of Diagnostic Oncology was published. As a result, it was felt that a fourth edition of this teaching textbook was justified. All chapters from the previous edition have been updated, many with new co-authors, and several chapters have been extensively revised to include new knowledge and data in molecular biology, genetic studies, and staging procedures. 
  • Two new topics have been added, the first to Chapter 7: “Cancer of the Gastrointestinal Tract” (Neuroendocrine Tumors by Dr. Matthew Kulke) and the second to Chapter 12: “Sarcomas of Soft Tissue and Bone” (Gastrointestinal Stromal Tumor by Drs. Suzanne George, Jason Hornick, and George Demetri). 
  • With the rapid progress in technology, specialization, and staging studies in the field of radiology, the topic of nuclear medicine has been expanded into a new and separate chapter organized by Drs. Annick van den Abbeele and Steven Burrell. 
  • Likewise, a new chapter, titled “Malignant Mesothelioma,” has been developed by Drs. Pasi Jänne, David Wu, and Lucian Chirieac, experts in this challenging and aggressive disease now seen with increased frequency worldwide. Malignant mesothelioma was previously covered in the Lung Cancer chapter, after initial discussion in the Sarcoma chapter. 
  • One other new chapter has been added to this Atlas—Chapter 20, “Complications of Cancer,” by Dr. Nadine A. Jackson and myself—to complement Chapter 21, “Systemic and Mucocutaneous Reactions to Chemotherapy”. 
Thus, the fourth edition of Atlas of Diagnostic Oncology has five new chapters and topics with a total of 24 new authors and coauthors, all with academic positions at Harvard Medical School and cancer experts at the Dana-Farber Cancer Institute, Massachusetts General Hospital, and the Brigham and Women's Hospital. 

The chapter format has remained the same: a general review of the cancer including incidence, epidemiology, etiology, and histopathology with molecular biology when relevant, and clinical features, which is followed by diagnostic studies and current clinical and pathologic staging. Most of the chapters use staging nomenclature from the sixth edition (2002) of the American Joint Committee on Cancer (AJCC). The seventh edition of the AJCC staging system will be published in late 2009. A new TNM staging system has been developed by the International Association for the Study of Lung Cancer (IASLC) in collaboration with the International Union Against Cancer (IUAC) and the AJCC and is included in Chapter 5. Detailed charts are used for histopathologic classification, diagnostic studies, and prognostic factors when important. Generous illustrations with detailed figure legends are utilized for examples of histopathology, staging, radiographs, CT, MRI, and PET images and differential diagnosis, along with examples of various clinical manifestations and relevant up-to-date references. As with previous editions, detailed treatment recommendations are not included since these are outside the goals of this book. The reader is referred to standard oncology textbooks and current review articles for details concerning management of cancer and hematologic malignancy patients. 

Of note, previous editions of this Atlas had 35-mm slide sets included for teaching purposes. With advances in computer technology it was possible to provide a CD-ROM with the second and third editions. With the fourth edition, a website will be available to download images of choice. These images have proven to be of great value for teaching medical and nursing students, house staff, and fellows. These images will also increase the scope of this Atlas by allowing for use in tumor boards, lectures, and other teaching applications. This fulfills our original goals of preparing a unique and illustrative educational Atlas of Diagnostic Oncology.

Foreword
As the field of clinical oncology moves forward, so must the published reference literature. The rate of new knowledge has accelerated and resulted in new diagnostic and therapeutic modalities. Understanding the unique biology of tumors that were previously included in major disease groups has resulted in their separation as distinct entities in the Atlas of Diagnostic Oncology –entities such as mesothelioma, gastrointestinal stromal tumors, and neuroendocrine gastrointestinal neoplasms. The updates, graphics, and comprehensive summaries have made this a classic in the past and assure its status in the near future. Nuclear radiology is an emerging field that has brought imaging into a new era. Imaging technology has improved the accuracy of staging and the evaluation of response. There is a new section that has brought this field forward and up to date.Dr. Arthur Skarin brings to this volume his commitment to visual and graphic aspects of medical oncology. He is a talented photographer and outstanding medical oncologist. He brings these talents together as editor of the Atlas of Diagnostic Oncology. In recognition of his talents, he has been a long-serving editor of “Images in Oncology” in the Journal of Clinical Oncology. I am particularly proud to write this foreword as 16 of the 21 chapters have authors who trained at the Dana-Farber Cancer Institute. This edition includes writers who are now on staffs of the Massachusetts General Hospital and Brigham and Women's Hospital. This edition will offer the clinician a ready reference to all the important issues in the daily practice of clinical oncology. This is indeed a multidisciplinary text with contributors from the related areas of surgery, pathology, radiology, and dermatology.     About the Author
    Arthur T. Skarin, MD, FACP, FCCP, earned his MD from the State University of New York at Buffalo in 1961, and received postgraduate training in internal medicine at the Millard Fillmore Teaching Hospital in Buffalo. After a fellowship in hematology and oncology at Boston City Hospital, he joined Brigham and Women's Hospital and Dana-Farber Cancer Institute (DFCI) in 1970. He served as Clinical Director of the DFCI Hematology Laboratory as well as Chairman of the Laboratory Committee from 1970 to 1998. He established the multimodality thoracic oncology clinic in 1975 and has served as Medical Director since then. During that time he also was chair of the human protection committee (IRB) for 12 years and became Associate Professor of Medicine at Harvard Medical School. He has been the Principal Investigator (PI) as well as site PI on numerous clinical research Phase I, II and III trials, both at DFCI Partners Cancer Care and the Cancer and Leukemia Group B (CALGB). He was appointed a Research Subject Advocate (RSA) in 2002 for NIH funded studies at the Brigham and Women's Hospital General Clinical Research Center (GCRC) and DFCI. Dr. Skarin is a board member of the American Journal of Oncology Review, the Journal of Cinical Oncology, Hospital Physician and also president of the Massachusetts Society of Clinical Oncologists.
    He has published 150 original articles, 97 abstracts, 67 chapters in books and 4 books.
