Of these, 220 images were rendered by medical illustrators. It will also be apparent that almost all previous black-and-white photographs have been replaced by those in vivid color. This is supplanted by a thorough canvass of the literature with stated efforts to emphasize evidence-based management. To do this, almost 2000 new journal and textbook references were added, and more than 400 of these were published in 2009.
