Hoffbrand: Color Atlas of Clinical Hematology 4th edition






Since the previous edition was published in 2000, major advances have occurred in the understanding of the molecular basis of hematological diseases. The World Health Organization (2008) Classification of Hematological Malignancies has incorporated this new knowledge to define many of these diseases and to add new entities. Molecular genetics are also now used more widely to classify benign disorders such as the thalassemias, inherited bone marrow failure syndromes, iron refractory iron deficiency anemia, hemochromatosis, and inherited coagulation disorders. We have attempted to fully illustrate this new knowledge in the fourth edition of the Color Atlas of Clinical Hematology. Scientific diagrams, images of cytogenetics, fluorescent in-situ hybridization (FISH), and gene arrays are included throughout. Fluorescent-activated cell sorting (FACS) and new imaging techniques, including FDG-PET, widely used in the diagnosis of Hodgkin and non-Hodgkin lymphoma, are also extensively included.
The two original authors have been joined for this edition by Dr. Paresh Vyas from the Weatherall Institute of Molecular Medicine; Dr. Vyas has taken responsibility for the first three chapters, which deal with the basic science of blood formation, including cell machinery, cellular aspects of hematopoiesis, and growth factors. We give only brief descriptions of therapy and have omitted the section from the previous edition on fine needle aspirate (FNA) appearances in nonhematological disease since FNA has now largely been replaced by tru-cut biopsy in hematological practice.
We thank the many colleagues throughout the world who have generously provided images. They are acknowledged on the individual figures. We particularly thank Dr. Elisabeth Nacheva and Dr. Wendy Erber for many cytogenetic, FISH, FACS, and hematopathology images, without which the book would have been incomplete. Professor Elias Campo came to our rescue when we needed many additional figures to comprehensively illustrate the new WHO 2008 classification of lymphomas, and we thank him and Professor Elaine Jaffe for giving us access to the provisional 2008 WHO classification prior to its publication. Professor Peter Isaacson and Dr. Alan Ramsay have also generously given many immunohistological images. We are sad that Professor David Mason, an outstanding hematopathologist and colleague, who was a major contributor to the Atlas in the third edition, died in 2008.
We are grateful to our Publishers and particularly Dolores Meloni, Michael Troy, Joan Sinclair, and John Ormiston for their unstinting help in assembling this vast amount of complex material into its final form. We are grateful to Jane Fallows for her expert scientific diagrams and the Department of Medical Illustration, Royal Free Hospital for expertise in assembling many electronic images into a publishable form. They have all been remarkably patient during our multiple additions and updating. We are also grateful to Avril van der Loo, who expertly typed and retyped the majority of the chapters.
We hope this book in its printed and electronic versions will be used as a comprehensive, up-to-date illustrated encyclopedia of blood diseases. We would be grateful to receive offers of new images for future editions and also advice if any errors or omissions have occurred in what has been an immense task.
