Sleisenger and Fordtran's Gastrointestinal and Liver Disease 9th Edition (Expert Consult Series)






The ninth edition of Sleisenger and Fordtran's Gastrointestinal and Liver Disease is the first edition for which neither of the founding editors is serving as a principal editor—Dr. Fordtran stepped down as editor after publication of the fifth edition, and for this edition, Dr. Sleisenger is serving as consulting editor. In Dr. Sleisenger's place, one of us, Dr. Lawrence J. Brandt, has become a principal editor. Given the explosion of information in gastroenterology over the past 2 decades and the incorporation of liver disease into the textbook since the sixth edition, we believe that the book is best served by three editors, rather than two. That the book continues to serve as a state-of-the-art, user-friendly source for the spectrum of gastroenterologic and hepatic disorders is a tribute to the vision and efforts of its founding editors, John Fordtran and Marvin Sleisenger. The sixth edition was dedicated to Dr. Fordtran, and this edition is dedicated with equal admiration, appreciation, and affection to Dr. Sleisenger.

As with previous editions, the editors have relied on the gracious advice of reviewers, colleagues, and trainees and have incorporated many improvements and refinements into this edition. Every effort has been made to minimize redundancies, correct deficiencies, and improve the book's flow and organization. Completing an enhancement that was begun in the seventh edition, virtually all endoscopic and histologic figures are now in full color. The color figures are fully integrated with the text in each chapter, not segregated into a separate atlas section as in many other textbooks. To be certain that all figures are of the highest quality and clarity, we were assisted by an imaging editor, Dr. Kenneth R. McQuaid, whose contribution we gratefully acknowledge. Two gastrointestinal pathologists, Drs. Pamela Jensen and Edward L. Lee, helped individual authors and editors find selected classic histologic examples of various diseases. In addition, for the eighth edition, nearly all of the diagrams and algorithms have been redrawn in a uniform and visually appealing style that complements and enhances the text.

The overall organization of the book remains similar to that of previous editions, although several chapters on new topics have been added and the placement of individual chapters has been refined. Moreover, fully one third of the contributing authors are new to this edition, thereby ensuring that the information is fresh, updated, and critically assessed; each contributor was chosen because of his or her expertise in the area covered and reputation for clarity and effectiveness as a writer. Since publication of the seventh edition in 2002, there has been a remarkable amount of new information in the field of gastroenterology; for example, at least 20 new drugs, including tegaserod for constipation-predominant irritable bowel syndrome, adefovir and entecavir for hepatitis B, and oxaliplatin, bevacizumab, and cetuximab for metastatic colon cancer, have been approved by the U.S. Food and Drug Administration. Every effort has been made to ensure that the content of the book is as current as possible.

Section I contains three chapters that deal with the Biology of the Gastrointestinal Tract and Liver and provides a cogent introduction to basic science relevant to gastroenterology. Basic pathophysiology also is incorporated into the chapters on specific diseases. Section II, as in the previous two editions, presents an Approach to Patients with Symptoms and Signs and includes chapters written by new authors, including Dr. DeVault (Symptoms of Esophageal Disease), the two Drs. Malagelada (Nausea and Vomiting), and Drs. Patel and Lembo (Constipation), as well as the stalwarts from previous editions. New authors for Section III on Nutrition in Gastroenterology—Drs. Mason, DeLegge, Becker, and Look—have joined veteran Dr. Klein to produce a veritable book-within-a-book that now includes separate chapters on Eating Disorders and on Obesity; the chapter on Food Allergies by Dr. Sampson also has been moved to this section. Section IV, Topics Involving Multiple Organs, has been expanded and reorganized. A new chapter has been added on Gastrointestinal Stromal Tumors (GISTs), by Dr. Demetri, and some chapters previously covered elsewhere in the book have been placed more appropriately in this section, including chapters on Eosinophilic Disorders of the Gastrointestinal Tract by Drs. Khan and Orenstein; Gastrointestinal Carcinoid Tumors and the Carcinoid Syndrome by Drs. Anthony and Kim; Vascular Lesions of the Gastrointestinal Tract by Dr. Brandt; Surgical Peritonitis and Other Diseases of the Peritoneum, Mesentery, Omentum, and Diaphragm by Drs. Fischer and Matthews; and Radiation Injury to the Gastro-intestinal Tract by Drs. Cho and Antoine. Single chapters by Drs. Jain and Thiele and by Drs. Reinus and Riely now cover Gastrointestinal and Hepatic Manifestations of Systemic Diseases and Gastrointestinal and Hepatic Disorders in the Pregnant Patient, respectively, and an expanded chapter by Drs. Jensen and Norton covers Endocrine Tumors of the Pancreas and Gastrointestinal Tract. Dr. Kimmey concludes the section with a thoughtful review of Complications of Gastrointestinal Endoscopy.

The next several sections constitute what we like to refer to as the “organ recital” of gastrointestinal and liver diseases: Sections V through X cover the Esophagus, Stomach and Duodenum, Pancreas, Biliary Tract, Liver, and Small and Large Intestine, respectively. Each section begins with a consideration of anatomy, histology, embryology, and developmental abnormalities, as well as basic function of the organ (for example, motility and sensory function of the hollow organs and biochemical function of the solid organs). The highlights and new contributors are many. Among the changes since the seventh edition are incorporation of the discussion of nonsteroidal anti-inflammatory drug–induced gastric ulceration into the chapter on Peptic Ulcer Disease by Drs. Cryer and Spechler (in addition to the chapter on Helicobacter pylori by Drs. Graham and Sung); creation of an expanded chapter on Treatment of Peptic Ulcer Disease by Drs. Chan and Lau; revision of the chapter on Acute Pancreatitis by Dr. Steinberg; updating of the chapter on Gallstone Disease by Drs. Browning and Sreenarasimhaiah and of the chapter on Acalculous Cholecystitis, Cholesterolosis, Adenomyomatosis, and Polyps of the Gallbladder by Dr. Persley (both chapters ably written in previous editions by our friend and colleague the late Dr. Lyman Bilhartz); and revision of the chapter on Sclerosing Cholangitis and Recurrent Pyogenic Cholangitis by Drs. Tung and Kowdley. New authors in the section on the Liver include the two Drs. Roy-Chowdhury (Liver Physiology and Energy Metabolism); Drs. Ahmed and Keeffe (Liver Chemistry and Function Tests); Dr. Stevens (Vascular Diseases of the Liver); Drs. Carithers and McClain (Alcoholic Liver Disease); Dr. Reid (Nonalcoholic Fatty Liver Disease); Drs. Shah and Kamath (Portal Hypertension and Gastrointestinal Bleeding); and Dr. Fontana (Acute Liver Failure). The single comprehensive chapter on Viral Hepatitis in the seventh edition has been divided into five separate chapters, which take advantage of the expertise of Drs. Sjogren, Perrillo, Nair, Berenguer, Wright, Krawczynski, Aggarwal, Heathcote, and Feld. In addition, a separate chapter on Liver Disease Caused by Anesthetics, Toxins, and Herbal Preparations, by Dr. Lewis, has been split off from the chapter on Liver Diseases Caused by Drugs by Drs. Teoh and G. Farrell.

New authors in the section on Small and Large Intestine include Drs. Kahn and Daum (Anatomy, Histology, Embryology, and Developmental Anomalies of the Small and Large Intestine); Drs. Harrell and Chang (Intestinal Water and Electrolyte Transport); Drs. O'Mahony and Shanahan (Enteric Bacterial Flora and Bacterial Overgrowth); Dr. Buchman (Short Bowel Syndrome); Dr. Giannella (Infectious Enteritis and Proctocolitis and Bacterial Food Poisoning); Drs. Kelly and Lamont (Antibiotic-Associated Diarrhea, Pseudomembranous Enterocolitis, and Clostridium difficile-Associated Diarrhea and Colitis); Drs. Su and Lichtenstein (Ulcerative Colitis); Drs. Fox and Stollman (Diverticular Disease of the Colon); Dr. Talley (Irritable Bowel Syndrome); and Dr. Camilleri (Acute and Chronic Pseudo-obstruction).
Finally, the last section of the book, on Psychosocial Factors in Gastrointestinal Disease, includes a new author, Dr. Locke (A Biopsychosocial Understanding of Gastrointestinal Illness and Disease), and new chapters on Palliative Medicine in Patients with Advanced Gastrointestinal and Hepatic Disease by Drs. Marr and Weissman and on Complementary and Alternative Medicine Therapies in Gastrointestinal and Hepatic Disease by Drs. Hass and Brandt.

As in previous editions, a mini-outline with page citations is included at the start of each chapter, and each major section contains a listing of the chapters with page citations in that section. The high-quality glossy paper for which the book is known has been retained, as have extensive cross-referencing and a comprehensive index.

The eighth edition of this classic textbook is a living tribute to its founding editors, Drs. John Fordtran and Marvin Sleisenger. True to their vision, the eighth edition presents a critical overview of the state of gastrointestinal practice and its scientific basis by eminent authorities in their respective fields.
