In the five years since the last edition there have been significant advances in most areas of intensive care medicine as the specialty has continued to evolve. This has included continued insights into both normal biology and disease, translation of this knowledge to the bedside, and results from large clinical trials directly relevant to clinical practice. Concurrently, health care professionals, patients and their relatives have unparalleled access to new knowledge through the internet. The original manual sought to provide useful and usable information at the bedside by distilling the immense amount of available information into a relevant and practical format. The burgeoning information highway that is the web does present a real challenge to a new edition of Oh’s Intensive Care Manual but it also mandates this edition, which seeks to make that information more readily usable at the bedside. Well-known and well-respected clinicians have once again produced an edition that presents a current framework with the most up-to-date contextual references which will be an important and practical contribution to the practice of intensive care medicine.
Many chapters have been extensively updated, and a number of new authors have faced the difficulty of summarising their field and choosing the key new references. While trying to prevent the manual from becoming any larger, the increasing importance and contribution of large clinical trials required a specific chapter. However, given the vast area of knowledge covered by the manual, there will be times when the reader wants further detail. The sixth edition will have been a success if it provides a clear framework for additional detail, and if it cites most of the key references. Indeed, this has been the successful format of previous editions that has made them so useful. More importantly, the sixth edition will have been worthwhile if it continues to make a positive contribution to the care of the critically ill.
