Hysteroscopy has developed rapidly over the last few years. The technique has moved from being a purely diagnostic ‘inpatient’ modality to a diagnostic and therapeutic ‘outpatient’ procedure. Advances in endoscopic technology have driven this paradigm shift. At international, national and local levels, the potential for developing outpatient interventional services in order to help meet healthcare delivery targets is being increasingly recognized. Desired targets include increasing the range, access and quality of services, improving patient choice and user experience, making progress in high priority areas (e.g. cancer) and developing the capacity to deliver these plans. Thus in the context of strategic planning in gynaecology, outpatient hysteroscopic services are likely to become increasingly prominent. The opportunity to meet these needs, by setting up outpatient diagnostic and treatment services (‘see and treat’ centres), should now to be seized.
The purpose of this book is to provide clear, unambiguous information about the capabilities of modern outpatient hysteroscopy and related gynaecological interventions. We have attempted to produce the first complete guide to diagnostic and therapeutic outpatient hysteroscopy, the so-called ‘see and treat’ approach. Current sources of written information do not deal with the practicalities of outpatient operating and are not generally ‘user friendly’. Operating in the conscious patient in an outpatient setting presents many new challenges. We describe strategies and provide tips to overcome these obstacles and thus speed the reader along the ‘learning curve’.
This book covers all aspects of modern day outpatient hysteroscopy and is based on our personal experience backed up by the available evidence. The book is divided into three sections. The first describes the fundamental theoretical and practical requirements of outpatient hysteroscopy including diagnosis and details on setting up and running services.
The middle section discusses frequently encountered presenting complaints and provides rational approaches for management. The final section addresses operative outpatient hysteroscopy and related interventions. Clinic set-up, pain relief, available technologies and specific operations are described as well as training, safety and medicolegal issues. The book aims to be concise while remaining comprehensive, and a consistent format is presented. An evidence-based approach is adopted throughout the book and summary ‘evidence boxes’ are provided at the end of each chapter along with key learning points. Where possible, the direction of future practice and research is highlighted in this dynamic field. By providing contemporary evidence-based approaches to managing substantial parts of benign day-to-day gynaecological practice in a concise manner, we envisage that the book will be of interest to a wide audience:
