Despite three decades of rapid growth and development, the specialty of Emergency Medicine (EM) continues to evolve more quickly than other medical specialties. The body of knowledge that an active emergency physician must command—and that a resident must learn—has increased dramatically in recent years. Examples of developments and events that have fueled this period of rapid growth in the EM knowledge base and patient demand include:
  • Several published research trials have identified new interventions that improve outcomes for patients presenting in the emergency department with acute cardiovascular or infectious disease and psychiatric and trauma-related illnesses or injuries. Recent technological advances in pharmacotherapy, medical imaging and laboratory measurement have contributed to making these improvements in patient care possible. Increasing emphasis on clinical practice guidelines for certain acute conditions assist emergency physicians who are managing several patients in a busy emergency department.
  • Hospital closings, in the face of increasing demands for access to emergency care, have resulted in the overcrowding of many emergency departments. In addition, insufficient inpatient beds has resulted in delays in transferring the sickest patients to critical care settings. The increasing numbers of uninsured, the insufficient number of primary care providers in some areas, and the aging of the population as a whole suggest that the demand for emergency care will not soon abate. As hospitals are forced to expand their emergency departments, the departments are being redesigned to accommodate more and sicker patients for longer periods. Observation Units and Chest Pain Centers are being added to larger hospital emergency departments to improve efficiency and capacity. Emergency physicians are learning to adapt to the increasing scope of care they must provide as a result of these environmental and demographic changes.
  • Events such as the World Trade Center attack in New York City on September 11, 2001, and the January 12, 2010, Haitian earthquake, have focused the attention of the media and the public on the critical role that emergency physicians and emergency departments play in disaster response. The EM subspecialty areas of disaster medicine, emergency medical services, and toxicology have experienced substantial increases in support for the development of rapid response capability and the performance of research in areas that will lead to improved management of terrorist threats.
The editors of this textbook have attempted to focus on the components of medical assessment and treatment that will assist the emergency physician functioning in a clinical setting by serving as a reliable source of clinical information. An attempt has been made to address significant recent advancements that are likely to alter the way the experienced emergency physician practices. This textbook is also intended to be helpful to residents and practicing emergency physicians who are assessing their need for further education, or who are preparing for an examination. Moreover, this book should also be of interest to medical students interested in emergency medicine, nurse practitioners, nurses practicing emergency medicine, and paramedics who are seeking to expand their body of knowledge in EM.
Finally, the editors are devoted emergency physicians committed to the practices of emergency medicine, the advancement of the specialty within organized medicine, and the support of research efforts that will improve the care provided to emergency patients.
