Myers: Operative Otolaryngology: Head & Neck Surgery 2nd edition 2-Volume Set






We are very pleased and proud to present the Second Edition of Operative Otolaryngology: Head and Neck Surgery. This book was written by members of our Department of Otolaryngology together with our colleagues in the Departments of Dermatology, Neurological Surgery, Ophthalmology, Oral and Maxillofacial Surgery, and Plastic Surgery, all from the University of Pittsburgh Medical Center. We have a long record of excellent relationships with our colleagues and without them we would not have been able to become leaders in subspecialty areas requiring intense multidisciplinary collaboration such as head and neck surgery, surgery of the cranial base, and neuro-otology.
In reviewing the table of contents from the First Edition, I was astonished to see how many of the procedures we described are now either completely obsolete or rarely used. The specialty of otolaryngology has evolved dramatically over the decade since the publication of the First Edition. We are now in an era in which very exciting new developments in technology have set the stage not only for new and improved surgical techniques but also for maintaining an excellent quality of life for our patients.
The Second Edition has 131 chapters compared with 125 in the First Edition. However, many of the original chapters were eliminated or consolidated under other headings so that we have been able to add 30 new chapters while only having to add an overall total of 6 chapters. Forty-six new authors have played a major role in producing this book. These new authors are all on the faculty of the University of Pittsburgh School of Medicine and bring great vigor and talent to those chapters that reflect their special areas of interest. Exciting new work in the field of otology includes the new techniques for BAHA surgery and updates the fast-moving field of cochlear implantation. Functional endoscopic sinus surgery was too new to be included in the previous edition. Here, these techniques are described in detail, as is the management of many of the vexing complications encountered with this type of surgery. The improvement in the quality of life of these patients is remarkable.
Great emphasis has been placed on the endonasal endoscopic management of tumors at the base of the skull and the unique complications encountered. In our institution, we have pioneered the two-team approach, which includes an otolaryngologist/head and neck surgeon and a neurosurgeon working simultaneously to allow two-hand dissection. Almost all tumors, both benign and malignant, in this anatomical area can be removed endoscopically. Transnasal endoscopic surgery also allows for decompression of the optic nerve in patients with Graves’ disease, traumatic injury, or orbital/skull base turmors. This technique can be done quickly with no side effects and the immediate return of vision is quite dramatic. Transnasal dacryocystorhinostomy is a helpful technique in the management of tearing. Cerebrospinal fluid leaks of all sizes are now managed successfully endoscopically.
The endoscope now plays an increasingly important role in removal of tumors of the thyroid and parathyroid glands. These techniques, in select patients, provide a bloodless surgical field with the possibilities for great exposure and complete removal of small- to medium-size tumors. The scarring is minimal and this technique has been well received.
The innovative use of the endoscope coupled with the CO2 laser allows for the excision of both benign and malignant tumors of the larynx. This technique was not included in the previous edition but is well described herein. The selective neck dissection, which wasn't widely used at the time of the First Edition, is also described here.
In the last ten years, we have developed one of the leading centers for the care of the voice. The University of Pittsburgh Voice Center has three fellowship-trained laryngologists in conjunction with a cadre of well-trained speech-language pathologists who provide care for the professional voice. They have contributed the entire section on laryngology, which is all new to this edition. Since many patients require Botox injection for various spasmodic conditions in the larynx, our laryngologists have also become quite expert in the technique of injection of Botox in the management of Frey's syndrome, another addition to the book.
The operative techniques described in this book are augmented with detailed information regarding preoperative evaluation and postoperative care since the philosophy of our department is that preoperative evaluation and postoperative care play important roles in the outcome of the procedure.
We've dedicated this book to our department, our colleagues, and our patients, who are the beneficiaries of these advances in surgery in our field. We hope our readers will benefit from the innovative advances that have taken place in this field and are described in this book.
