Like its four predecessors, this book provides an in-depth presentation of the basic science and physiology of normal and complicated obstetrics followed by chapters describing the most updated, evidence-based information on normal obstetrics and high-risk obstetrics. The efforts of a new team of associate editors, the contributions of more than 30 new authors, and a new publisher have enabled us to greatly enhance the content and access to the information in this fifth edition. Four carefully chosen new associate editors, Drs. Henry Galan, Laura Goetzl, Mark Landon, and Eric Jauniaux, each bring their own expertise and insight to this fifth edition. The addition of an international editor from London, Dr. Jauniaux, has enabled us to expand the coverage of this text to include information on clinical care from abroad.
Life-long learning has become essential for the best practice of obstetrics. Our readers will find 14 new chapters, many added to reflect important changes in our knowledge base since the fourth edition in 2001. These new chapters include placental anatomy and physiology, immunology of pregnancy, diagnostic techniques for prenatal diagnosis, management of abnormal labor, operative vaginal delivery, cervical incompetence, preterm premature rupture of the membranes, amniotic fluid disorders, thyroid and parathyroid diseases, adrenal and pituitary disorders, thromboembolic disorders, sexually transmitted diseases, viral infections, and psychiatric disorders. An appendix of normal laboratory values has been added to accompany the appendix on anatomy.
This fifth edition includes more than 30 authors who have joined us for the first time, and we greatly value the new information they have brought to our readers. We also want to recognize the 29 outstanding authors who have continued to contribute. A special tribute is due to 10 leaders in our specialty who have written chapters in every edition of our textbook: Drs. George J. Annas, Richard L. Berkowitz, D. Ware Branch, Sherman Elias, Timothy R. B. Johnson, Mark B. Landon, Adam A. Rosenberg, Phillip Samuels, John W. Seeds, and Baha Sibai.
We greatly appreciate the support of our new publisher, Elsevier, who has joined us for this fifth edition. We have synergistically made significant changes that should benefit our readers. Illustrations are now in full color. Each chapter provides a list of key abbreviations that will facilitate reading. A paginated outline appears at the beginning of each chapter making it easier to locate information on specific topics. The edges of the pages for the two appendices have been colored so that they can be quickly located.
We recognize the outstanding efforts of Melissa Dudlick, Judith Fletcher, Rebecca Gaertner, Stephanie Donley and their team at Elsevier. In addition to the Elsevier staff, the editors have had the invaluable editorial and secretarial support of Robyn Cosby, Nancy Schaapveld, and Belinda Felder. Because this fifth edition will be online and will be continually updated, this entire team will continue to work closely with the editors.
Nearly 25 years ago, the three of us met to discuss our plans for the first edition of Obstetrics: Normal and Problem Pregnancies. Like most medical specialties, obstetrics has since become more complex and challenging. Yet, we continue to feel strongly that the practice of normal and high-risk obstetrics remains among the most fascinating and rewarding in medicine and nursing. We believe that this fifth edition will not only serve as an inspiration to those considering a career in obstetrics and gynecology but will also be a valued resource for those who have already joined us in this exciting field.
